Vital Statistics

 

The Laredo Vital Statistics office hours of operation are Monday through Friday 8:00 a.m. – 5:00 p.m.

All Birth and Death records requested are issued within the same day.

the Same Death Record

Requirements to obtain a Birth or Death Certificate

Qualified applicant

  • Self/Registrant (person on record)
  • An immediate family member (parents – name of parent must be listed on birth record; grandparents - birth certificate of one of the parents will be required to prove relationship; siblings – birth certificate will be required to prove relationship)
  • Spouse (a marriage license will be required)
  • Legal Guardian (copy of the legal guardianship papers, ie. court order is required)
  • Legal Representative (document establishing legal interest required)

Proper identification is required:
Identification must be Current/Valid

  • US Government issued driver’s license
  • US Government issued State/City/County ID card
  • US Student ID with picture
  • US/State/City Government employment badge or card
  • Prison ID / Offender ID card
  • US Military ID
  • Passport (from any Country)
  • Border Crossing card (Passport Card)
  • US Citizenship Certificate or Certificate of Naturalization
  • Permanent Resident Card or Resident Alien Card

Copy of current Identification for applicant is needed.  Acceptable types of identification are listed above.


Send a money order for the type of certified copy being requested plus $6.23 for mailing of certificate by certified mail.

Amendment to a Birth or Death Certificate

 

The most common change made to a birth certificate is the addition of the father’s name.  This change to a certificate can be made by submitting the Application for a New Birth Certificate Based on Parentage Form (VS-166) (http://www.dshs.state.tx.us/vs/reqproc/amendparentage.shtm)signed by both parents in the presence of a notary public with one of the following documents:

·         Certified copy of a marriage license between the mother and the biological father

·         Properly completed State of Texas Acknowledgement of Paternity Form (VS-159.1)

·         Certified copy of a court decree establishing parentage between the child and the biological father
